Code of Malacath has Necromancy shunned, so I don't think I can become a lich. Though my court mage is a necromancer, would that work?
And I have extended my lifespan once, but now I seemingly can't do it again because I have a "attempted extend lifespan" flag.
With necromancy I'm not too sure if you can command your mage to cast it on you. Still if it is possible then he needs to have mastery of necromancy.
With restoration you need to wait until the decision is active once more. You should get a small pop up. If not then check it every do often
About how long does it take to get the decision again?
I don't know specificaly but it's usualy when you are in the halve of your extended life. You should have a buff or debuff that states when it's due.

I do not unfortunately, all I have is the standard aging debuff.
I do see on the decision that I need to not have a specific character flag, but I cannot tell when I will lose that character flag.
