Yes there are nations that play as the UK or control the isles. But in general, the default setting you should approach the world with is that you exist on the normal worldmap. Meaning, IRL countries exist.
Yes, every inch of the world is controlled by someone. Claims can overlap though. And thus, every inch is controlled by someone just as it isn't at the same time.
